I'm sure a good bespoke tailor would be more than willing to design a neckline to accomodate the second head - I mean, what's a personal tailor for if not to design a perfectly individual suit?!
Somehow, though, I don't think that white fur thing Zaphod was wearing was bespoke - I'll have to compare to the definition at the site! :P
There's also the extra arm. And, according to Mark Wing-Davey, who played Zaphod in the original radio series and the TV series, the character had another spare appendage as well ... the trousers he wore on the TV series had two flies (one zipped and one buttoned). ;-)
